Medical Care Agreement
		A flat retainer based fee agreement.
		We do not bill for any services provided by our P3 practice.
		Patients are expected to maintain health insurance for hospitalizations, specialist consultations and services, and catastrophic care. The patient will also be responsible for the cost of laboratory tests and diagnostic studies.
		An interview will be held with each potential patient. This includes a face to face meeting between the physician and the prospective patient. It is important to ensure a mutual partnership between the doctor and patient.
		Each patient will receive a written and signed contractual agreement between the P3 practice and the patient.
